Care Coordination
Our friendly and experienced Resource Counselors can assist you with things such as: housing, rent assistance, utility assistance, transportation, etc.
As part of Community CarePort, our Care Coordinators provide “Care Traffic Control” to break down silos and improve client health. They work one-on-one to help identify risk factors in all aspects of a client’s life and help them access needed services including healthcare, housing, education, employment, and more.
